Instructions For Printing and mail-in-entries
1. Click on the entry button.  
2. Print entry form
(Right Click on the entry to print or hit CTRL P on your keyboard)
3. Fill out your entry.

4. Make a Copy for your records.
5. Then Mail in your entry with payment and your health papers.

6. Please, no changes to your entry after sent in.
